Athletic Training Forms	1. ATS Forms (Online) Pre-participation HIPPA Release Injury and Illness Reporting Assumption of Risk Concussion Education Sheet 2. Concussion Vital Signs (Online) 3. Physical Form (Printed, Physical must be completed within 6 months of start date) Sickle Cell (Printed)	In the past, people have not filled out all the forms necessary for ATS, or attached a photo of their insurance card. Instructions for that are here: Uploading insurance card into ATS The CVS test must be done in one sitting with no distractions, allow for approximately 45 minutes. The test must also be taken on a computer with a mouse. A track pad will produce an invalid test result. You are not eligible until you have a "valid test". Upload on ATS E-Files. Compliance Paperwork	An email will be sent to your Oswego email address with directions on how to fill out the online compliance forms	The email is usually sent in early August. Please respond to the email within a day or two.
Compliance Paperwork	International Students Only: International Amateurism and Eligibility Form (Printed)	All international student-athletes need to meet with Associate Director of Athletics Heather Moore before they can be cleared to participate.
